About the Role:
We are looking for Social Workers with significant recent experience in undertaking NHS Continuing Healthcare Decision Support Tools (DSTs). You will play a crucial role in ensuring individuals receive appropriate care and support, working collaboratively with health partners and families.
Key Responsibilities:
* Conducting comprehensive Care Act Assessments prior to DST meetings.
* Undertaking Mental Capacity Assessments and best interest decision-making.
* Completing NHS Continuing Healthcare Decision Support Tools (DSTs).
* Commissioning appropriate care packages for individuals no longer eligible for CHC.
* Liaising with healthcare professionals, families, and other stakeholders.
* Maintaining accurate and up-to-date records using the Adults Liquid Logic (LAS) IT system.
* Ensuring compliance with the NHS Continuing Healthcare Framework and relevant legislation.
Essential Requirements:
* A recognised Social Work qualification (e.g., Degree in Social Work, DipSW).
* Current registration with Social Work England.
* Significant and demonstrable recent experience in undertaking NHS Continuing Healthcare related work, including DST completion.
* Thorough working knowledge of the NHS Continuing Healthcare Framework.
* Practical experience and proficiency in using the Adults Liquid Logic (LAS) IT system.
* Strong assessment and analytical skills.
* Excellent communication and interpersonal skills.
* Ability to work autonomously and as part of a multidisciplinary team.
