About the Role
We are seeking to recruit an enthusiastic Care Coordinator to join Bath and North East Somerset, Adult Social Care, First Response Team.
* You will respond to initial contacts to the team, wherever possible providing information and advice to enable people to facilitate the support that they need.
* You will gain an initial understanding of needs and triage to identify people who may require further involvement from adult social care.
* You will coordinate and assist with a range of services under close supervision of professionally qualified practitioners and managers.
The role is hybrid with attendance required at our offices in Peasdown St John 3 days per week.
About You
* You must hold level 3 NVQ or equivalent and have experience of working within adult social care.
* You must be able to communicate skilfully over the telephone and be able to use a range of IT applications, including the Council’s social care database.
