We have an exciting opportunity for an Occupational Therapist to join the South Falls Team, Midlands Partnership Foundation Trust, in Staffordshire and Stoke on Trent. We are looking for highly motivated individuals, who are keen to make a difference in this rapidly evolving clinical area. The successful applicant will have the opportunity to influence the development of this service alongside the operational lead, clinical leads, and clinical colleagues.
The team provides high-quality assessment to patients across Staffordshire who present with falls. You will be part of the Allied Health Professions (AHP) undertaking a Physiotherapist role within the South Falls Service providing specialist assessment to patients who have fallen. This includes providing advice and onward referral for therapy/diagnostics. This assessment work is predominantly provided within patients' homes; therefore, the ability to travel across the Staffordshire area is essential.
Working on your own as a registered practitioner within the established multidisciplinary team, you will undertake a community Occupational Therapy role for the South Falls Team, supporting the other specialist teams as demand requires. You will be expected to assess, identify, and agree goals and treatment plans using diagnosis and evidence-based treatment programmes to meet a wide variety of patient needs in the most appropriate location in the community e.g., patients' own homes, clinics, care homes, and day centres. The role will involve supporting the team to provide training on falls prevention on an ad hoc basis at the request of Clinical/Ops Lead. The role will involve delivering high-quality, professional, personalised, and compassionate care that reflects the trust's values.
