Job Title: Registered Staff Nurse

Working hours: Full time | hours per week | Monday to Friday | hrs per day between the hours of 08:30 and 16:30
Salary: £30,000 per annum

Purpose of the Post
To provide the highest standards of end-of-life patient-centred care to patients requiring specialist palliative care in our day hospice, within patients homes and other community settings, developing your skills and knowledge to meet the needs of our patient group.

Main duties and responsibilities
18. Independently assess physical, psychological, spiritual, cultural, religious, social, legal and ethical issues affecting the patient and carer.
19. Proactively engage in the development of services.
20. Plan and deliver appropriate care in consultation with the multi professional team.
21. Facilitate communication and co-operation with other health care multidisciplinary teams and with both statutory and voluntary agencies.
22. Be aware of changing policies, practices, and innovations within nursing and palliative care to promote evidence-based clinical intervention, care and support ongoing developments at KEMP.
23. Support and supervise, Student Placements, Care Services volunteers, and support workers across the hospice and in the community.
24. Provide flexibility in role and approach, dependent upon patient and organisational needs.
25. Act as infection control link.
26. Working in partnership and supporting the Care Services Team, plan, implement and evaluate care from first assessment to discharge or death.
27. Ensure patients views are taken into account when planning care by establishing their care needs and preferences both on admission and continually thereafter, including identifying advance care planning needs.
28. Demonstrate practice that is evidence based and up to date.
29. Use professional knowledge and experience to influence decision making to improve outcomes for patients and the team.
30. Keep accurate clinical records in accordance with KEMPs policy and procedures, and ensuring all data is recorded accurately on the clinical management software system.
31. Participate in collaborative decision making with all those responsible for delivering care within the Care Services team to ensure that patients and carers wishes and preferences are represented in every aspect of their care.
32. Perform clinical procedures competently according to KEMPs policies, procedures and practice; provide nursing and personal care as required.
33. Maintain accurate patient electronic records .
34. Negotiate and advocate on behalf of patients and carers and recognise the need to refer to and liaise with other agencies and professionals.
35. Maintain patient confidentiality, advocacy and respect patients rights and wishes regarding treatment and care.
36. Participate in reflective practice/clinical supervision.
37. Work in accordance with NMC Code of Professional Conduct.
38. Assist the Care Services Manager in meeting and maintaining the standards set by the Care Quality Commission.
39. Contribute to the supervision and support of the Care Services volunteers.
40. Take appropriate action to record, monitor and report complaints, accidents and untoward incidents, in line with hospice policy.
41. Welcome all visitors to KEMP, promoting good patient care and both internal and external relations.
42. Ensure safe custody and administration of drugs in accordance with Hospice policy.
43. Assist with the orientation and induction of new Hospice staff.
44. Be willing to prepare for and lead morning handover and MDT meetings.
45. Participate in the ongoing development of our care services in day hospice and the community in conjunction with colleagues.
46. Participate in and promote education and training for staff, volunteers and visiting students both formally and informally.
47. Maintain personal professional development pertinent to current role and linking own development needs with KEMPs appraisal and objective setting process.
48. Promote the use of evidence-based practice.
49. Comply with mandatory training and other training mutually agreed with the Head of Care.
