Requirements:
* Care Coordination : Ensuring all aspects of a patient’s care are organized and communicated across providers.
* Health Assessments : Regular evaluations of the patient’s health status.
* Personalized Care Plans : Tailored plans to meet the specific needs of each patient.
* Support Services : Access to social services, mental health support, and community resources.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
