We're seeking a dedicated Hospital Liaison Worker to provide a hospital-based service in Kent. You will work in close partnership with Accident & Emergency teams to develop and implement care plans for frequent hospital attenders.
Using the Making Every Contact Count (MECC) approach, you will offer advice & information, harm minimization and brief interventions.

Full Time Salary: £27,449.52 - £31,529.41

Responsibilities
* Work in close partnership with Accident & Emergency teams to develop and implement care plans for frequent hospital attenders.
* Provide specialist assessment of patients attending/admitted to hospital with alcohol-related issues using AUDIT screening tools and provide brief interventions and onward referral for patients.
* Provide advice, support and training to hospital staff regarding drug and alcohol awareness, screening, assessment processes and referral pathways.
* Develop and facilitate education/training opportunities to enhance the knowledge and skills of clinical staff working with this group.
* Work closely with community substance misuse teams to ensure assertive and effective follow up of repeat presentations and discharges from the hospital.
* Keep GP's informed of patient’s status and discharge plan/referrals within 5 days of patient’s hospital discharge.
* Develop specific links with identified specialist acute services.
About You:
To thrive in this role, we're looking for someone who has:
* Experience of working with the Making Every Contact Count (MECC) approach, offering advice & information, harm minimization and brief interventions.
* The ability to manage challenging behavior in a calm and professional manner.
* The ability to deliver bite-sized training sessions to professionals.
* Experience of working sensitively in a hospital setting.

Salary Range (pro rata if part time)
CGL points 23 to 28 (£27,449.52 - £31,529.41)
