An exciting opportunity has arisen for a Lead Practice Nurse to join our Network, 30-37.5 hours per week based at the Grove Surgery Christchurch.
Main duties of the job
The ideal candidates will have recent experience within a Primary Care setting, be adaptable, highly motivated and relish a challenge. You will be part of a dynamic and innovative team who value teamwork, professional practice, mutual support and a friendly working environment.
The post holder will be an experienced Practice Nurse who will be a role model to team members. The post holder will demonstrate a high level of professional and clinical practice and leadership and work in accordance with the NMC Code. As the Lead Nurse for the nursing team, the post holder is accountable for nursing service delivery. They will lead and manage all the nursing resource, working closely with the Practice Supervisors and GPs to deliver the practice priorities. Clinically, the focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. The Lead Nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership and direction for the nursing team.

JOB TITLE: LEAD PRACTICE NURSE
REPORTS TO: NURSE MANAGER
Key responsibilities:
1. Delivering a quality service
2. Recognise and work within own competence and professional code as regulated by the NMC.
3. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
4. Lead on infection prevention and control as defined in the practice health & safety policy and the practice infection control policy and published procedures.
5. Immunisation Lead, maintaining expertise on national immunisation program. Accountable for the process of ordering, delegation and monitoring vaccines and other medicines.
6. Prioritise, organise and manage own and others workload in a manner that maintains and promotes quality.
7. Deliver care as an individual and team according to NSF, NICE guidelines and evidence-based care, assessing effectiveness of care delivery through peer review, team meetings, benchmarking and formal evaluation and leading on the maintenance of quality governance systems for the nursing team.
8. Implement and review the application of evidence-based practice in nursing.
9. Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
10. Evaluate patients response to health care provision and the effectiveness of care.
11. Lead and participate in shared learning across the practice and wider organisation and assess the impact of policy implementation on care delivery.
12. Monitor the performance of the GPN team in accordance with local policies.
13. Work with Practice Supervisors to ensure Practice Nurse levels are consistent and authorise requests for annual leave and training leave.

Clinical practice
1. Assess, plan, develop, implement, provide and evaluate treatment programmes including, but not limited to: Health promotion and well-being plus individual plans for patients with/at risk of a long term condition.
2. Manage and treat long-term conditions, in line with national and local policies and practice needs.
3. Review medication for therapeutic effectiveness, in accordance with evidence based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments.
4. Prioritise health problems, intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
5. Support and manage health needs of women presenting for family planning and cervical cytology consultations.
6. Implement and participate in vaccination and immunisation programmes for both adults and children including patients travelling abroad.
7. Meet the needs of patients presenting for opportunistic wound care.
8. Diagnose and treat minor illnesses.
9. Work as an effective and responsible team leader, supporting others and exploring the mechanisms to develop new ways of working.

3. Ensure safe storage, rotation and disposal of vaccines and drugs.

Qualifications
* Qualifications:
* Registered Nurse with a valid NMC (Nursing and Midwifery Council) registration.
* Evidence of continued professional development.
* Experience:
* Significant experience in primary care.
* Experience in managing long-term conditions such as diabetes, asthma, and COPD.
* Leadership or supervisory experience in a healthcare setting.
* Skills:
* Strong clinical skills in areas such as infection prevention and control, health promotion, and chronic disease management.
* Excellent communication and interpersonal skills.
* Ability to work autonomously and as part of a multidisciplinary team.
* Proficiency in using healthcare IT systems like SystmOne or EMIS.
* Additional Qualifications:
* Management or leadership training.
* Specialist qualifications in chronic disease management (e.g., diabetes, COPD, asthma).
* Experience:
* Experience with clinical audits and quality improvement initiatives.
* Skills:
* Ability to lead and mentor a team of nurses and healthcare assistants.
