Job summary
The Flow Co-ordinator will play a prominent role in ensuring the delivery of flow
within the Elderly Care Wards. This will be achieved through developing excellent
internal communication pathways, the utilisation of the internal escalation process
and ensuring the multi-disciplinary team adhere to the Department's internal
professional standards.
The post holder will ensure the timely flow of patients through the area to help
identify available capacity and place patients appropriately, working within agreed
procedures and protocols.
Main duties of the job
Monitor the transport process and identify any delays or issues that the
clinical team are experiencing.
Act as the first point of contact for users of the service and to deal proactively
with all enquiries, welcome and orientate patients and their relatives/friends to
the clinical area.
Work as part of a dedicated team working closely with staff at all levels and
communicating effectively within the department/units.
Deal with telephone enquiries professionally ensuring, where possible that the
caller's needs are met or they are transferred to the most appropriate person.
Knowledge of daily bed capacity issues via site capacity team.
Attendance at MDT huddles.
Liaise with Site Team as and when appropriate.
Ensure that all staff are aware of any information that will assist in any safe transfer/discharge.
Liaise with all members of the multi-disciplinary team.
Promote all transfers/discharges within agreed timeframes.
Identify bottlenecks in the system which prevents the smooth and timely
transfer of patients
To co-ordinate the transfer of patients to allow sufficient capacity and safe
placements of patients.
Liaise with staff in Elderly Care Wards, and other UHB hospital to facilitate
appropriate patients transfer
