Purpose
To assist the Care Co-ordinator in their day-to-day duties, ensuring a smooth and high-quality service is delivered to the carers, their families, and people with care needs, in line with the aims, policies, and standards of the organisation.
The post will require fulfilling rostering, administration, supervisory, and staff/client liaison duties.
To support the entire client journey from referral to service end.
In the absence of the Care Co-ordinator, the Service Support will temporarily cover all duties required by the Care Co-ordinator.

Generic Responsibilities
* To be accountable to the designated Care Co-ordinator but in absence be flexible to support all the Care Co-ordinator team.
* To identify available and appropriate care staff in accordance with our personalised service and roster effectively and efficiently to ensure delivery of high-quality services.
* To update and implement personal care plans, and ensure the smooth running and effectiveness of the care package.
* Participate in the “on call” rota.
* Visit clients in their own homes to undertake service reviews.
* Supervise and observe Care Support Workers working with clients, providing witness statements to support the attainment of Diplomas.
* Conduct at least 2 supervisions for each care support worker under line management of the designated Care Co-ordinator per year.
* To organise and monitor the Care Co-ordinator’s diary.
* To accept referrals in line with Service Level Agreements and Crossroads Care policies and procedures.
* To communicate with clients and staff as required.
* To update IT systems with up-to-date and relevant information.
* To administer the care journey for a client from referral to service end.
* To track, achieve and report on assigned KPIs.
* To manage the alerts and take appropriate action.
* To signpost internally and externally where appropriate to the correct service.
Deputy Care Co-ordinator Responsibilities
In the absence of the Care Co-ordinator you will be required to fulfil the following:
* To investigate any reported accident/incident, ensuring that all Health and Safety legislation is met and the Operations Manager is informed of any incidents that have arisen.
* To monitor referrals in accordance with Crossroads Care policies and procedures, contribute towards weekly reports subsequently issued to the Operations Manager detailing capacity, referrals, clients, staff issues, comments and complaints, and health and safety.
* To monitor and manage the day-to-day demands made upon the service.
* To ensure the service delivered meets the requirements of the service user and is within budgetary constraints.
* To supervise and appraise care staff in line with the organisations policies and procedures.
* To raise awareness of the complaints procedure and where action is necessary, report immediately to the Operations Manager/Senior Care Co-ordinator.
* To ensure the training needs of all care and support staff are identified and fed into the development of the annual training plan.
* To ensure all clients referred are assessed for eligibility.
* To undertake a home visit to devise a personalised care plan and to determine, in consultation with clients and relevant professionals, the support to be provided by Rotherham Crossroads Care in accordance with the Health & Safety legislation.
* To undertake risk assessments in client homes following appropriate referrals.
* To identify care staff training needs as part of the client needs assessment process and liaise with the Training Officer to organise appropriate training.
* To develop a high-quality personalised service to ensure independence is maintained and service users are able to remain living in their own homes.
* To ensure all associated administration is in place through liaison with the office staff.
* To assist and enable the service user to access other relevant support services.
* To maintain regular contact with clients by telephone/home visits to regularly monitor and review the effectiveness of the care package.
* To be available on a flexible basis in periods of client/carer emergency including emergency cover, emergency care set up, and on call.
Allocation of Work
* To organise and allocate regular weekly schedules for CSWs in geographical region.
* To organise and allocate cover work for CSWs in geographical region.
* To plan and schedule annual leave for CSWs.
* To monitor, manage and process the unallocated visits for geographical region.
* To match care staff to clients to provide person-centred services.
* To email timesheets to care staff each week.
* To communicate any changes to care to staff and clients.
* To preserve and ensure client continuity for CSWs.
* To schedule training, staff meetings, and supervisions into the roster for CSWs.
* To monitor and manage salaried CSW hours to ensure hours are fulfilled.
* To notify clients of any changes to their visit, e.g., time, CSW.
* To action all alerts for the team and take appropriate measures.
* Action change in tasks on electronic call monitoring.

* To ensure compliance with relevant regulatory requirements both internally and externally.
* To work closely as part of a team, providing flexible support and cover to maintain high-quality services.
* To ensure a quality service is provided by monitoring standards and reviewing the service.
