The post holder will assist the (Nurse) Manager in delivering a 24-hour Nurse led respite/enhanced support service. They will work in collaboration with the multi-disciplinary team in the assessment, planning, implementation and evaluation of comprehensive care plans and care packages. The post holder will mostly work without direct supervision and they will be responsible for leading and supporting the team in providing good quality care on night shifts
An adaptable and flexible approach is required due to frequent changing needs of the SU and service delivery.
Managing the staff and allocating them to the units at night
Monitoring the service users health at night, following care plans and risk assessments
Escalating health concerns when managing deteriorating health from epilepsy and diabetes or breathing complications
Completing night logs and monitoring paperwork
To Ensure that medication and ILD equipment bag are audited at night and escalating where the equipment as expired or needs replacing.
managing a small case load - sometime working days to complete home visits and paperwork in care files.
Participate in service audits.

The successful candidate will be responsible for:
• leading shifts
• a specific caseload (Named Nurse responsibilities)
• the assessment and treatment planning for individuals in conjunction with other healthcare professionals
• the formulation and implementation of care plans and risk assessments in conjunction with parents/carers and any significant others involve
• the operational admission and discharge of individuals
• ensuring staff under their supervision are competent and have access to each individual care plan
• ensuring medicines are administered appropriately and timely, and adhere to the Partnership Trust’s and the Service’s medicine policies (including controlled medication, PRN, Rectal buccal and injections
• ensure all recording is accurate and timely
• ensure reports are made available for reviews and meetings as required
• liaising with parents/carers and other professionals
• performing and training of clinical procedures, PEG/NG feeding, oxygen and suction therapy, SATS, heart rate monitoring
• Identifying own learning needs
• ensuring your practice and that of others is in accordance with the NMC guidelines, CQC standards and Coventry & Warwickshire Partnership Trust policies and guideline
• Infection Control, Health and Safety and Fire regulations and procedures to be adhered to
• working, on occasion, at an alternative location within the Partnership Trust to meet the needs of the service
