Supporting the Band 6 and Band 7 nurse specialists provide advice and care for a defined group of patients following current pathways. Face to face clinic and telephone consultations. Assist in the remote monitoring of patients including coordinating appointments, blood tests and radiology procedures. Supporting patients with medicines management which includes communicating with hospital, community pharmacies along with homecare delivery companies.
