Primary Responsibilities:
• Acts as a role model in confidentiality, professionalism, appropriate attire,
• and work attitudes.
• Promotes and practices Community Hospice mission, vision, and values.
• Assists the Registered Nurse (RN) in the development and provision of the patient’s Plan of Care and recommends change/revision as necessary.
• Assists the IDG team in performing specialized duties, and procedures, and administers medications, as appropriate.
• Utilizes an individualized, outcome driven approach in the delivery of patient care.
• Communicates and updates the RN and physician in observed changes in patients’ condition, and provides written documentation as required.
• Assists the patient/caregiver in learning appropriate self-care techniques.
• Participates in and assists with discharge planning.
• Participates in IDG meetings and collaborates with team members in the identification, management, and/or resolution of the patient/family/caregiver needs/problems.
• Maintains the highest standards of professional conduct in relation to information that is confidential in nature by maintaining patient confidentiality and privacy in all aspects of care. Shares information only when the recipient’s right to know is clearly established according to HIPAA regulations.
• Processes physician orders: electronic, written, or verbal.
• Maintains and orders medical supplies as requested.
• Facilitates communication with the attending physician and if applicable the hospice physician when appropriate; (e.g. upon death of patient, and/or transfer of patient to another level of care.)
• Communicates and collaborates with all team members.
• Assists in the disaster preparedness and is available for assigned duties.
• Engages in the administration of intravenous therapy (IV), within the scope of LPN licensure.
• Participates in Performance Improvement activities, as applicable.
• Adheres to general patient and life safety standards.
Qualifications:
Education:
1. Current Licensure as a Licensed Practical Nurse/ Licensed Vocational Nurse in the state of Florida.
2. IV Certification required. State of Florida preferred.
3. Current CPR Certification.
4. Current State of Florida Driver’s license with driving record acceptable to Community Hospice’s insurance carrier
Experience:
5. A minimum of two (2) years of patient care experience required.
6. Acute care nursing experience preferred.
7. Previous hospice nursing experience preferred.
8. Graduate from an accredited Florida Board of Nursing program for LPNs/LVNs.
Knowledge of:
9. Appropriate educational principles and methods as applied to hospice nursing.

10. Able to respond effectively in the treatment of chronically, acutely, and terminally ill clients and their caregivers.
11. Ability to work flexible hours as required to meet identified patient needs.
12. Ability to adapt to changing organizational needs.
13. Speaks, writes, reads, and comprehends English.
14. Excellent organizational, interpersonal, telephone, documentation, and communication skills.
15. Must possess strong patient observation management and documentation skills.
16. Ability to function in care, regardless of the patient’s age, race, creed, color, disability, financial condition, or sex of the patient or caregiver.
17. Ability to use sound judgment and integrity to handle the confidentiality of patient assignment and care.
