Job summary An exciting opportunity has arisen for the post of Service Planning Manager within the North Powys Wellbeing Programme delivered jointly with the Local Authority. The post holder will play a key role in providing planning expertise to support the development of a new transformational model of care in North Powys. This project will act as a flagship to the implementation of a 'seamless new model of care' under the 'A Healthier Wales: Our Plan For Health and Social Care, and will require the post holder to stay informed about and contribute to communities of learning about these new models of care as they develop across Wales. The Service Planning Manager will report directly to the Programme Assistant Director, transformation. Main duties of the job The role will involve working with clinicians, managers, staff, professionals, partners and other key stakeholders to: Undertake mapping of existing services and development of a detailed case for change to support business case development. Develop new models of care, service specifications and pathways in conjunction with health and care professionals, based on an agreed planning framework, national guidance and legislation, local population needs assessment and Powys' Health and Care Strategy and Area Plan. It will require a significant amount of engagement and communication messaging The post holder will work closely with the Project Director and work stream leads to provide technical planning expertise to support transformational change and capital business case developments for North Powys. The post holder will play a key role in capital business case development from strategic outline business to full business case (FBC) for a proposed new health and care facility which will be designed to deliver the model of care and service specifications developed by the project. They may also contribute to the development of other Local Authority and Health Board plans and business cases to support the broader implementation of the new model of care.
