Referral and Assessment Manager – Business Development Team
Location: Work from home with extensive travel to locations in the North West & Midlands
Rate of pay: £35k
A Full UK Car License is essential for this role.

This role is to support with:
* The management of all new enquiries and referrals made to Aspirations.
* Completion of Assessments.
* Support with the implementation of new packages of support.
The Person – Main Duties
* Respond to enquiries and referrals made to Aspirations in a professional and timely manner.
* Ensure necessary reports to track all referrals and enquiries are completed and updated within required timeframe.
* Assess needs of individuals and their prospective support packages, making recommendations regarding necessary requirements for needs to be met by Aspirations.
* Manage contact and relationships with the individual, professionals, and key stakeholders involved with the individual and their assessment, transition, and admission until point of first review.
* Monitoring and ongoing evaluation of care and support throughout the review process for identified case load and defined timeframe.
* Re-assessment of existing placements whereby the placement is at risk, making necessary recommendations to ensure needs can continue to be met within the business.
* Ensure that all assessments are completed, and all reports prepared within prescribed time frames, comprehensively and to a high standard.
* Ensure a safe admission into the service by working in partnership with the operational team and key stakeholders to design and implement a person-centred transition plan.
* Ensure all legal frameworks are adhered to in relation to assessed needs and package of support.
* Work in partnership and support operational colleagues with the development of initial Care Plans and Risk Assessments.
* Where identified and allocated, support with post-incident review and lessons learned/debrief to provide oversight, guidance, and recommendations.
* Provide a pre-agreed visit facility to discuss support element within care and support planning to support service delivery and ensure appropriate representation at forthcoming meetings.
* Work alongside the quality team to ensure best practice guidance is advised/observed within the remit of the role on behalf of Aspirations.
* Where identified/assigned, provide support to meetings, decision making, and review as a representative of Aspirations.
* Deliver necessary training where appropriate as identified in needs led assessment.
* Document all relevant movement/tracking of admissions, risk, and discharge activity within the assigned area/location/geography.
* Active involvement in budget setting for assigned geography locations.
* Work to agreed budgets and manage KPI’s against this with regards to occupancy.
Qualifications
* The role must be maintained by a practitioner who has expertise and relevant experience within the field of health and social care: Specifically Learning Disabilities and/or Mental Health – operational delivery of a support service is ideal.
* The person should be qualified to Degree level or equivalent in a relevant subject, e.g., RNLD, RNMH, Social Worker, Health and Social Care, Occupational Therapy, Psychology; this list is not exhaustive, and all relevant subjects will be considered.
* Where necessary, the person should be registered with the relevant professional body; this must be maintained as active as part of the role.
Location and travel
* A full driving license and access to a vehicle is required.
* Travel and overnight stays would be expected as part of the role.
* The role will be predominantly based within an identified geographical location.
* This may change dependent on the needs of the business and as assigned by the line manager.
* Presently the geographical region will cover the Midlands and North West, although this is not exhaustive, and some national travel may be required.
