Job Title: Social Worker – Transitions Team
Location : Cheshire West
Rate : £35 per hour
Liquid Personnel is currently recruiting for a Social Worker to join a busy Transitions Team based in Cheshire West.
What will your responsibilities be?
You will be expected to work within organisational, policy, and legislative requirements, including the Care Act, Mental Capacity Act, and the Mental Health Act. A good working knowledge of safeguarding, risk assessment, and enablement is essential. A person-centred approach to transition planning is necessary to help young people and their families prepare for the journey into adulthood, so you must be able to work closely with families and young people, promoting their views and wellbeing in all interventions.

Qualifications and Experiences:
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
