** NOTE - Two Year Secondment ** An exciting opportunity to work in the cancer division at UHSussex as a personalised care project manager who will support the coordinators and existing teams to establish pathways and enable progress to be made in improving equitable access to information and support and access to holistic needs assessments. The Macmillan project manager role will enable smarter and more agile working practices of the existing Cancer Support Worker pool in addition to removing Personalised Stratified Follow up responsibilities from Clinical Nurse Specialist and Cancer Support Worker teams to improve provision.

Main duties of the job
1. The role would initially support the continued roll out of Personalised Stratified Follow Up at University Hospital Sussex which would ensure our support worker team are then able to focus on providing patients with appropriate information, support and Holistic Needs Assessment completion.
2. Support the implementation of stratified follow up pathways within the trust in line with local and national guidance, enabling patients to self-manage.
3. Develop dashboard that is produced on a monthly basis and produce briefing documents/ papers as requested to present at internal and external meetings where appropriate.
4. Be responsible for programmes of work that improve the experience of patients living with and beyond cancer at University Hospitals Sussex Trust.
5. Monitor the Personalised Cancer Care agenda, including Holistic Needs Assessment, Personalised Care Plans, End of Treatment Summaries and Health and Well Being Events.
6. Provide advice on issues in relation to personalised care interventions, policies, protocols and service development. To provide managerial and professional leadership of a defined team of staff in order to deliver the personalised care agenda.
7. Collaborate with the Macmillan Horizon Centre services to ensure an equitable offer of access to personalised care across University Hospitals Sussex Trust.

Job responsibilities
1. Streamlining Holistic Needs Assessment completion and recording including use of Electronic Holistic Needs Assessment via My Health and Care Records/Patient Knows Best.
2. Development of Information and Support services both via Macmillan Hubs and via Patient Knows Best, and in collaboration with the Macmillan Horizon Centre.
3. Promote equality of care across geographical locations and tumour sites across all University Hospitals Sussex Trusts.
4. Liaise with relevant AHPs e.g. physiotherapist, dietician in the development of Health and Well Being events.
5. Ensure Holistic Needs Assessment becomes part of the cancer Clinical Nurse Specialist/ support workers core tasks.
6. Review core themes from care plans and engage with patients and carers to identify any gaps in care provision.
7. Improve signposting to cancer information and support/ health and wellbeing events to improve patient self-management, working with the Band 4 Cancer Support Worker.
8. Relieve pressure on the tumour specific teams enabling them to focus on the delivery of specialised and complex care, under guidance of the personalised lead.
9. Promote the involvement of patients, carers and their families, embracing co-production and co-design across all workstreams. Use feedback from people with lived experience to influence the service design and delivery of personalised care. Ensure that clinical teams and key stakeholders are aware of the outcome/ patient experience.
10. Assess inequalities in access to personalised care at University Hospitals Sussex and work in partnership with voluntary and charitable organisations to improve this.
11. Support service redesign, workforce planning and lead on business case development to support and embed effective and sustainable change.
