We are seeking a first contact physiotherapist to join our expanding team.
Working hours: 37.5 hours per week, full time (part time applicants will also be considered)
Annual Salary £46,148 - £51,500 dependent on experience
Main duties of the job
We are seeking to recruit a motivated and experienced HCPC registered physiotherapist with proven clinical skills. The successful candidate will be comfortable working autonomously as a first contact physiotherapist. They will work in our 3 practices and have a named GP for support and mentoring within each practice. They will possess excellent communication and team working skills.
The FCP will:
1. Provide clinical expertise, acting as first-contact physiotherapist and making decisions about the best course of action for patients' care.
2. Deliver programmes of supported patient self-management.
3. Request investigations when necessary to facilitate diagnosis and choice of treatment regime.
4. Refer to CCG commissioned therapy and specialist services when needed.
5. Provide an advisory role to other members of the clinical team and support our educational processes.
6. Administer corticosteroid joint injections and request imaging.
About us
North Care Network is a recently formed Primary Care Network (PCN). We consist of 3 like-minded North Lincolnshire practices based in Barton-upon-Humber, Brigg, Winterton and the surrounding villages. We are responsible for the care of around 33,884 patients. We have benefitted from a first contact physio service in our practices for a substantial period of time and wish to maintain this excellent service which has been highly valued by our patients.
Job responsibilities
The physiotherapist will be primarily based within the practices that constitute the network but may need to work from other premises within the local area as determined by the network's clinical director and management team.
JOB PURPOSE
1. Provide clinical expertise, acting as first-contact physiotherapist and making decisions about the best course of action for patients' care (including in relation to undifferentiated conditions).
2. Progress and request investigations to facilitate diagnosis and choice of treatment regime, understanding the information limitations derived from these and the relative sensitivity and specificity of particular tests diagnostic services such as x-rays and blood tests and interpret and act on results to aid diagnosis and the management plans of patients.
3. Deliver programmes of supported patient self-management, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that minimise the need for pharmacological interventions.
CORE RESPONSIBILITIES
1. Takes professional responsibility as a first-contact physiotherapist, with high-level decision-making and clinical-reasoning skills to assess, diagnose and triage patients.
2. Manages a complex caseload (including patients with long-term conditions, comorbidities and multi-factorial needs).
3. Leads, manages and contributes to service delivery.
4. Accountable for decisions and actions via HCPC registration, supported by a professional culture of peer networking/review and engagement in evidence-based practice.
5. Streamlines pathway of care by providing a responsive service so that patients receive timely access to care.
6. Ensures care is proactive, preventive in focus and population-based, with an emphasis on early intervention.
7. Provides care which is tailored to the individual needs.
8. Supports patients to set their own goals and be confident in their approach to self-management.
9. Communicates effectively and appropriately with patients and carers complex and sensitive information regarding diagnosis, pathology and prognosis.
10. Uses a range of clinical skills which may include non-medical independent prescribing, joint / soft tissue injections, and joint aspirations.
11. Provides learning opportunities for the whole multi-professional team within primary care.
12. Works with MDT to develop more effective and streamlined clinical pathways and services.
13. Exercises professional judgement, making, justifying and taking responsibility for decisions in unpredictable situations, including in the context of incomplete/contradictory information.
14. Manages interactions in complex situations, including with individuals with particular psychosocial and mental health needs and with colleagues across the primary care team, sectors and settings.
Person Specification
Experience
* Evidence of leadership and management expertise and knowledge acquired through CPD and experience.
* Knowledge and experience of using standardised assessments and development of relevant outcome measures.
* Knowledge and understanding of national legislation, guidance and regulatory framework including NSF, NICE, Information Governance, Equality and Diversity, Data Protection, Safeguarding, Professional and Clinical Guidelines.
* Knowledge of Clinical Governance and implications for service area.
* Evidence of continuous improvement through benchmarking and sharing and utilising best practice.
* Awareness and understanding of relevant health and Social Care legislation.
Qualifications
* Diploma/Degree in Physiotherapy.
* Current HCPC Registration.
* Current full driving licence.
* Post-graduate qualifications relevant to MSK & Orthopaedic speciality to Masters level or equivalent.
* Non-medical prescribing or willingness to complete.
* Membership of Chartered Society of Physiotherapy.
* Management/leadership qualification or equivalent.
* Completed recognised IT course e.g. ECDL/Elite/MOST.
