The post holder will co-ordinate the Dynamic Support Registers for Calderdale and Kirklees place. The aim of the DSR is to bring a person-centred and individualised approach to ensuring that the care and treatment and differing support needs of the person and their families are met, and that barriers to progress are challenged, overcome, and analysed, options compared and reviewed to agree the best way forward. The DSR Co-ordinator would be responsible for bringing relevant professionals together to collectively discuss and agree a plan. The post holder is not responsible for making any care or clinical decisions but will gather relevant information into a central point. The post holder will chair Kirklees and Calderdale Dynamic Support Register update meetings from which will populate/update information onto the Dynamic Support Register (DSR) from information provided by key workers across various agencies and provide data to relevant Department/Agencies. The post holder will be responsible for horizon scanning and keeping up to date with national guidance and embedding this into systems and processes to ensure a robust Dynamic Support Register to support the needs of the local population. The post holder will provide education, planning and deliver training around DSR processes supporting by a standard operating procedure for learning disability and/or autism services across the Calderdale and Kirklees partnership. Responsible for providing data and information to meet national reporting requirements and in support of the Transforming Care Partnership Board via a monthly DSR report which will also need to be linked with CTR delivery.
