About the Bridge Care
The Bridge provides community based registered care services for people with learning disabilities, autism and associated complex care needs. We provide personalised outcome-based support to enable individuals with significant care and support needs to live safely, happily and successfully in the community. Our focus is to fill the gaps in accommodation and support provision for uniquely complex and challenging individuals by taking a fresh perspective to their support needs. We provide bespoke individual accommodation choices that are entirely flexible to meet individual needs. Everyone’s opinion matters to us and we see this as key to the success of the people we support. Position
The Support Worker will play a key role in delivering high-quality, person-centred care to adults with learning disabilities. This role involves working closely with internal teams, families, and external professionals to ensure that individuals are supported to lead fulfilling and independent lives. You will be involved in the development and implementation of tailored care plans, ensuring that the needs of the people we support are met in line with regulatory and organisational standards.
Key responsibilities:
* Personal Care and Support: Assist in providing care to adults with learning disabilities, autism, and other complex needs, supporting them to live as independently as possible.
* Empowering Individuals: Encourage and support individuals to achieve their personal goals and participate in a range of meaningful activities, promoting independence and dignity.
* Person-Centred Approach: Provide care that is tailored to the individual’s needs and preferences, ensuring that every person receives respectful and dignified support.
* Safeguarding and Compliance: Ensure that care and support are delivered in line with regulatory requirements and company policies, safeguarding the health, safety, and well-being of the people we support.
* Team Contribution: Contribute to creating a supportive and collaborative environment, where everyone works together to deliver high-quality care.
Your Responsibilities:
* Build positive, respectful relationships with the people we support, encouraging them to take an active role in decisions about their care.
* Provide personal care as needed, always respecting the individual’s dignity and privacy.
* Assist individuals with daily living tasks, including personal hygiene, meal preparation, and mobility.
* Support individuals in engaging with meaningful activities and social events, promoting emotional, physical, and psychological well-being.
* Monitor the health and well-being of those you support, ensuring their needs are met and reporting any changes to the senior team.
Delivering High Standards:
* Follow individual care plans and risk assessments, ensuring all care is delivered safely and to the highest standard.
* Administer medication as prescribed and ensure that it is recorded accurately and safely.
* Support individuals with healthcare appointments and assist them in managing their medical needs.
* Encourage participation in recreational activities, ensuring each individual has the opportunity to develop new skills and interests.
Teamwork and Communication:
* Maintain clear and accurate documentation on the needs and progress of the people we support.
* Work collaboratively with the team, ensuring effective communication and sharing of information about care needs.
* Respect the confidentiality of individuals and share information only as appropriate, in line with company policies and legal requirements.
* Support colleagues and contribute to a positive and respectful working environment.
Health and Safety:
* Follow all health and safety policies and procedures, ensuring a safe and supportive environment for both staff and the people we support.
* Report any safeguarding concerns, incidents, or health and safety issues promptly.
* Take responsibility for maintaining your own safety and well-being while on duty.
Professional Boundaries and Communication:
* Communicate effectively and respectfully with colleagues, families, and external professionals to achieve positive outcomes for the people we support.
* Represent the service in a professional manner and act as an ambassador for the company.
* Report concerns about care quality or colleague performance promptly and in line with the organization’s safeguarding and complaints policies.
Sustainability and Compliance:
* Ensure that care is delivered in line with relevant legislation and best practice guidelines, including the Mental Capacity Act and safeguarding protocols.
* Contribute to the recruitment and onboarding process as needed, supporting the growth of the team.
* Help maintain a positive, values-driven culture within the team, supporting training and development.
Minimum Qualifications: The role of Support Worker requires this position to have or be working towards a level 2 (Health & Social Care) diploma.
