Job summary This is an exciting opportunity for a Band 5 to be part of the CJLD (Criminal Justice Liaison & Diversion) team within NRP (Northumberland Recovery Partnership). The successful candidate will work within a dedicated team who work predominantly with those clients recently released or due to be released from prison, those who are involved with probation or any other criminal justice service, including integrated offender management programme (IOM). The successful candidate will also be involved in assessments of new clients in particular prison releases and those potentially starting on drug/alcohol treatment programmes. They will be expected to hold a caseload, and will help further develop relationships with the clinical side of NRP and the CJLD team and build better links external services. Main duties of the job To manage case work under direction of care co-ordinator/ Pathway Clinical Lead and provide ongoing Assessment and Recovery focused Care Planning for service users who do not have enhanced needs and where risks are assessed as low, under the supervision of the Pathway Clinical Lead. To provide, under the direction of a Care Coordinator and or Pathway Clinical Lead, effective treatment and intervention to service users with complex and substantial needs. To deliver evidenced based clinical and psychologically based Interventions, with appropriate Training and supervision and as directed by Pathway Clinical Lead and Supervisor. The post holder will work as a member of the multi-disciplinary team ensuring thathigh quality, individualised care is delivered and that this promotes Recovery andWellbeing and maximises Independence.
