We have an exciting opportunity for 2 full-time permanent Social Workers to become a part of our Neighbourhood Health and Social Care Teams. The successful candidates will join our community-based multi-disciplinary teams located within health centres in Blackpool. They will work collaboratively to ensure effective health, care, and support assessments and services are implemented for people and carers to remain well in the community.
You will need to be passionate about supporting our community in Blackpool, be committed to your role as a Social Worker, and keen to grow and develop in line with Social Work England protocols.
