Job summary
CYGNET Primary Care Network (PCN) is looking to recruit an innovative and enthusiastic Paramedic / Advanced Clinical Practitioner to work in primary care.
The successful candidate will deliver new and innovative services to meet the needs of our patients. You will work as part of a multi-disciplinary team as a key first point of contact practitioner for acute presentations. The role will deliver clinical care to patients, supporting clinicians, practice, and primary care staff within CYGNET PCN. We are adopting a holistic approach to assessment, diagnosis, and treatment.
Pay is negotiable depending on experience. The position is full-time and benefits from NHSPension scheme membership.

Main duties of the job
Key areas of work:
1. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
2. Take a comprehensive patient history, conduct physical examinations, and use expert knowledge and clinical judgment to identify the potential diagnosis.
3. Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the clinicians. Refer patients for investigations and/or the appropriate specialist where appropriate.
4. Use practical experience to plan and provide skilled and competent care to meet patients' health and social care needs, involving other members of the wider health care team as appropriate.
5. Liaise and seek advice from appropriate healthcare professionals.
6. Access to a vehicle and a full driving licence is essential for this post.

Cygnet PCN covers a large geographical area within the East Riding of Yorkshire. Our network covers over 52,000 patients and is made up of the five following GP Practices:

Our network comprises a range of roles from Clinical Director to Care Co-ordinator. PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Our PCN is proactively providing care and services for the people and communities we serve.
New roles are being introduced to the network as we are expanding into different areas of healthcare. The network provides a single point of access for the nursing, residential and learning disability homes in the area. All our team are passionate and committed to making a difference to patient care.

Job responsibilities
The post-holder will work across the CYGNET PCN demographic as an autonomous, accountable advanced clinical practitioner to assess, consult and treat patients in residential/care homes and in primary care settings. A variety of consultation skills, both face to face and remote, will be key to providing and supporting this cohort of our population.
The post-holder will be suitably qualified with a valid registration with HCPC or NMC. An independent prescribing qualification is desirable.
Patients will be triaged, assessed, and appropriately signposted in accordance with agreed rotas and systems. These will typically be patients presenting with a wide range of acute illness, urgent, same day requests for treatment and chronic disease management. You will be providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
