The role involves managing patients with acute and chronic conditions, delivering expert clinical care, and leading health promotion initiatives. Core Responsibilities: Manage frailty and chronic disease caseloads, developing and evaluating treatment plans. Provide specialist clinics and perform clinical procedures, including pathology collection and test interpretation. Lead vaccination and immunisation programmes, ensuring best practices in patient care. Recognise and refer mental health needs, safeguarding concerns, and substance abuse cases. Support medication management and contribute to practice targets (e.g., QOF). Liaise with external agencies to provide comprehensive patient support. Handle clinical emergencies and promote opportunistic health education. Leadership & Development: Oversee the nursing team, offering mentorship and guidance. Act as an audit lead, ensuring compliance with clinical standards. Drive service development, reviewing and adapting practices in line with national policies. Conduct significant event reviews and implement quality improvement measures. Develop specialist areas of interest and refine practice protocols to enhance patient care.
