South Warwickshire University Foundation Trust is seeking a highly skilled and experienced Lead Advanced Clinical Practitioner (ACP) to join our Speciality teams. The successful candidate will provide expert clinical care and leadership within the Emergency directorate. This role involves delivering senior clinical decision-making and contributing to the strategic development of advanced practice across the trust.
There are four roles available in speciality areas: Acute Medicine, Cardiology, Frailty and Respiratory.
The post holder will take a major role in providing the highest level of quality expert practice for patients, admission avoidance, new models of care and supporting discharge pathways. This role involves acting as a catalyst for change in practice and service development, challenging professional and organisational boundaries and generating new solutions that best meet the needs of patients and their families.
The post holder will ACP will have significant clinical experience and a proven record of leadership, change management and innovation.The post holder will work both collaboratively and autonomously, with authority, using advanced knowledge and skills, to help develop and deliver services across the Trust. The post holder will work at an expert level across the 5 recognised domains, these being:

Job responsibilities
Key Responsibilities: -
Advise on the Trust strategy for new models of care in conjunction with the General Manager, Clinical Director, Associate Chief Nursing Officer and Associate Chief Operating Officer for the Emergency Division and other Divisions as appropriate.
Clinical responsibility for acute pathways within a defined specialty.
The provision of expert clinical practice in regard undertaking highly specialist roles across the specialty, ensuring patients are enabled to be discharged earlier from hospital or preventing admission. This will include providing clinical leadership in clinical practice; providing professional and clinical leadership to the Advanced Clinical Practitioners, Professional leads and specialty nursing teams where appropriate; leading and participating in advanced Nursing/Therapist practice. The post holder will work in clinical practice for 50% of time.
Responsible for the interpretation of National policy in the development of services and Trust related outcome measures, policies, guidelines, protocols and procedures.
To contribute to the development of new models of care to transform the care provision within the Trust ensuring sound and effective clinical pathways are developed, implemented and evaluated.
To undertake research and audit activity in evaluating models of care and informing national policy.
To provide an expert consultancy service to patients, carers and colleagues, both within the Trust and across wider networks.
To provide professional leadership in a way that inspires colleagues, improves clinical standards and quality and develops professional practice.
To provide advice and leadership in the education, training and development for the service delivery across the acute, community and primary care sectors.
Person Specification
Qualifications
* First Level Nursing or Therapy registration
* Non-Medical Prescriber
* Evidence of previous multi-professional working
* Demonstrates a comprehensive understanding of clinical governance issues
* Demonstrates comprehensive appreciation of the NHS plan and Trust Objectives/Big Moves
* Evidence of previous involvement in research
Experience
* Significant experience at senior nurse or therapist level
* Significant experience of working within an Acute setting
* Evidence of previous input at strategic level
* Experience of implementing research based practice
* Experience in developing and implementing policies and procedures
* Experience in identifying and planning multidisciplinary training and development
* Evidence of successful change management
* Experience of working in a community setting
Skills
* Excellent oral, written and presentation skills
* Experienced in coaching and mentoring colleagues in a clinical setting
Personal Qualities
* Ability to influence and act at all levels within the organisation
