Job summary
The post holder will support in Primary Care to deliverpatient services and provide a specialist Paramedic resource for health careprofessionals and service users, working in collaboration with other members ofthe Multidisciplinary Team,following Heath Education England 2021 Roadmap to Practiceunder Additional Roles Reimbursement.
The post holder will work within the community, and worktowards becoming autonomous, accountable Paramedic Practitioner, in theprovision of a holistic approach for individuals including assessment,diagnosis and treatment, to deliver quality patient services.
Depending on experience, to assess, diagnose, treat, referor signpost patients/service users who attend surgery with undifferentiated orundiagnosed condition relating to minor illness or minor injury.
The post holder must own vehicle for home visits withexpensed remunerated in overall financial package.
Main duties of the job
1. Assessing and treating patients in their own place of residence and those attending surgery.
2. Diagnosing and treating patients presenting with minor illness.
3. Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team, secondary health care and MDT and/or organise admission to hospital if required.
4. Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
5. To undertake assessment for patients within the community and those attending the surgery, using diagnostic skills and initiation of investigations where appropriate.
6. To maintain accurate, contemporaneous healthcare records appropriate to the consultation.
7. Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high quality patient care.
8. Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the service.
9. Participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need.
10. Contribute positively to the effectiveness and efficiently of the team and work colleagues.
11. Take responsibility for safeguarding children and vulnerable adults and work with other team members to ensure they are protected.

The role of PCN Paramedic is to support primary care to deliver high quality patient services as part of a Multi-disciplinary Team (MDT) across our three GP practices. This may include Home Visiting, clinic working and patient caseload management.

The post holder will be required to:
12. Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs
13. Complete individual administration and professional responsibilities
14. Participate in the administrative and professional responsibilities of the practice team
15. Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
16. Ensure the clinical computer system is kept up to date, with accurate details recorded and amended
17. Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator
18. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
19. Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
20. Attend and participate in practice meetings as required
21. Restocking and maintenance of clinical areas and consulting rooms
22. Undertake pathological specimens and investigatory procedures

Qualifications
Essential
23. Active and relevant professional registration with HCPC
24. BSc or IHCD or above in Paramedic Science
25. Evidence of relevant CPD
26. Excellent communication skills
27. Basic IT Skills
28. Evidence of commitment to own personal and professional development
29. Ability to manage and prioritise workload
30. Ability to work as part of an integrated multi-disciplinary team
