Our Community Nurse Teams are responsible for the assessment of nursing needs, implementation and evaluation of care/treatment and the delivery of high quality holistic care within patients' own home, residential care and within our catheter and leg ulcer clinics. You will be expected to support your peers, the team leader and the organisation in promoting the Trust's values and behaviours.
The hours of work are between 08:00 and 17:00, over 7 days.
You will need to be able to work independently, possess good time management and organizational skills, and be expected to be innovative and flexible to work in other areas if required to meet service needs.
Please note whilst the main base will be in Northwich with the Firdale Team, you may be required to work across the footprint of Central Cheshire Integrated Care Partnership.
Travel is an essential part of this role; please demonstrate on your application form how you would meet this requirement.
Main duties of the job
We are looking for Band 5 staff nurses to support the Caseload holders in delivering high quality, efficient and safe care. We are looking for Nurses who are newly qualified, community experienced or have no previous experience; for our team it is about the individual.
We are looking for an enthusiastic person who possesses both clinical and digital skills to support in the promotion of quality improvement for a local population. Main duties will involve wound management, chronic condition management, catheter management, and palliative support and end of life care. Our patient groups are varied and often complex.

Job responsibilities
1. Work under the direction of the Community Nurse Team leader and be responsible for the assessment and management of the health needs of an identified population.
2. Support and maintain patients in their preferred place of care, enabling them to maximize their independence and quality of life.
3. Liaise with healthcare professionals and other agencies to ensure effective service delivery.
4. Assess, provide and review holistic nursing care, advice, and information to patients and carers.
5. Undertake initial assessments on receipt of specific referrals to the caseload.
6. Assess, plan, implement, and evaluate a plan of care to meet the patients' needs.
7. Provide skilled nursing care and undertake clinical tasks including wound care and medication administration.
8. Recognize, interpret, and analyze clinical data related to individual clients.
9. Demonstrate and supervise nursing care that can be safely delegated to the patient/carer.
10. Participate in health education of the individual and community promoting self-care.
11. Accurately record all patient contacts ensuring confidentiality.
12. Manage and assess patients' discharge potential and needs.
13. Ensure equipment and aids meet the needs of individual patients.
14. Participate in the delivery of care involving complex needs.
15. Refer people to other practitioners when necessary.
16. Prioritize and manage own caseload.
17. Participate in the measurement and monitoring of quality standards.
18. Maintain accurate personal and professional records.
19. Report accidents/incidents occurring while on duty.
20. Contribute to the co-ordination of off duty and holiday leave with colleagues.
21. Act in a manner which illustrates respect for privacy, dignity, and confidentiality.
