The teams consist of Community Nurses,Physiotherapists, Occupational Therapists, Healthcare Assistants and adminstaff with GP, paramedic and social care colleagues working closely alongside. Thisenables the teams deliver planned nursing, therapy and admissionavoidance/supported hospital discharge interventions to the adult patientpopulation of Redditch and the surrounding areas.
We are looking for a highly motivated, flexible team player with a strong,quality driven patient focus and an ability to work through change to embed newways of delivering care. Main duties of the job
The role willrequire:

Shifts 7.5hrs between 7:30am 18.00pm, Monday to Sunday including bank holidays). Additional enhancements will bereceived for weekend working.
A willingness to develop moreskills and commitment to learning.
A typical working day willinclude home visits for adults needing a range of nursing interventions. Theseinclude caring for people reaching the end of their life, managing their painrelief and other symptoms in a sensitive and compassionate manner, all types ofwound care- post operative wounds, leg ulcers, pressure ulcers and complexdressing regimes, IV therapy and other injectable drug administration andholistic assessment of a persons needs.
Back at your base, you will beinvolved in team meetings, goal/care planning and evaluation and handovermeetings. You will also support the teaching and learning of nurse studentspre and post registration.
