Our unique Acute Hospital at Home team are looking to recruit a motivated and caring Occupational Therapist. Our service proactively manages and monitors a defined caseload of patients. The team is truly multidisciplinary, and consists of trained Therapists, Staff Nurses, Therapy Practitioners and Therapy Assistants.
You will become part of a team that are in the privileged position of supporting people in their own home by setting rehab goals which enables patients recently discharged from hospital to return to their base line function. This is a dynamic process requiring thorough OT assessment at multiple stages of the patient journey revising care plans in response to your assessments.
Main duties of the job
We are looking for an Occupational Therapist who:
* Can assess and review patients and set realistic and achievable rehabilitation goals
* Has experience supporting patients to meet their rehabilitation goals.
* Can demonstrate an ability to think critically and respond to change in circumstances and challenging situations.
* Can supervise, motivate and develop a team of support staff, assistants and students.
* Has good IT skills to support with the running of the service and the essential communication and monitoring of patients which allows for successful transitions to other teams.
A full UK Driving License is an essential requirement for this role.

Job responsibilities
Occupational therapy will be delivered in accordance with the COT standards of practice and the Code of Ethics and Professional national guidelines.
Duties will include:-
* Undertake (or work towards) comprehensive assessments of patients including those with diverse or occasional complex presentations using advanced clinical reasoning skills and assessment techniques to establish a prompt and viable management plan.
* Formulate and deliver occupational therapy treatment plans based on sound knowledge and evidence based practice, evaluating patients' progress and tailoring treatments to meet their needs.
* Use a range of interpersonal skills such as motivating, negotiating, counselling and reassuring skills along with excellent communication skills to communicate often complex related information effectively.
* You will have freedom to act as an autonomous practitioner you are responsible for planning your own realistic and achievable treatment interventions with supervision as required from senior staff to enable the patient to regain skills.
* The patients you will be supporting will often be leaving hospital sooner than traditional models of care and at an earlier stage of the rehab process. Therefore, your skills will be needed to not only provide short term intervention but also for establishing sustainable and achievable levels of function that will help to minimise the risk of readmission to hospital

Education, Qualifications & Training
Essential
* To Have a Diploma or Degree in Occupational Therapy
* Registered as an Occupational Therapist with the Health Professions Council
* Evidence of other relevant and recent CPD/ training
Desirable
* Membership of relevant special interest group
* To have a full UK driving licence
Knowledge and Experience
Essential
* Recent, significant post qualification experience of working in the acute physical sector as a qualified therapist. General Medicine & Surgery, Orthopaedics, Elderly Care, A/E, Critical Care, Respiratory
* Min 2 Years experience in the core rotational skills including evidence of community working
* Special interest in care of the frail elderly
* Evidence of contributing to the development of Therapy assistants/ student therapists.
Skills & Abilities
Essential
* Ability to present information, written and orally, in a clear and logical manner
* Ability to keep accurate and legible clinical notes
* Ability to demonstrate clear depth of clinical reasoning and knowledge of OT assessment and interventions for patients' with varying medical conditions, commensurate with Band 6 level.
* Ability to work as an autonomous practitioner
* To show an understanding of clinical governance and its implication for the Occupational Therapy service
* To have Good computer skills
* To have ability to prioritise and manage caseload
* Ability to work within a multidisciplinary team
