This is a fixed term position of 12 months The shift pattern for this position is Mon- Fri, 9 - 5. This post offers the exciting opportunity to work as a Team leader within the Hospital Discharge team working closely with the two senior Social Workers in the integrated team. The role provides the opportunity to maintain clinical skills and manage and lead the Hospital Discharge health team, AHPs and admin staff. The successful candidate will be responsible for the operational management and service delivery within the Hospital Discharge team which includes managing and monitoring resources within a defined budget and acting as a clinical lead within the team ensuring a high quality person centred service to the patients that are delayed.
