Job Summary
We are looking for dynamic, passionate, and motivated nurses with experience in Community Nursing to be Team Leaders within the Camborne Community Nursing Team.
The post holder will work with the team to ensure the provision of a holistic nursing assessment and high-quality, safe, evidence-based care to patients and carers, aiming to maintain patients in their own homes. Our patients are at the center of all that we do, so we are looking for nurses who are calm, caring, compassionate clinicians interested in delivering the highest quality care and supporting our patients to lead full and independent lives while promoting self-management of their health conditions.
This will be an exciting and evolving job role within the Primary Care Network, exploring new multi-disciplinary ways of integrated working. Therefore, flexibility and a willingness to explore new exciting ways of working is a must.
This is a community-based role that requires frequent travel to various locations throughout the county, including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
Main Duties of the Job
As an autonomous practitioner, the post holder will be responsible for the triage, assessment, and treatment of patients with complex health and social care needs. They will manage a clinical caseload within a community nursing team with guidance from the Community Nurse Team Lead, ensuring the provision of a holistic nursing assessment and high-quality, safe, evidence-based care to patients and carers.
The post holder will work in this adaptable and evolving role, focusing on safe, effective admission avoidance and supporting timely discharge from hospitals and other care settings. They will implement personalized care plans and collaborate with the whole community resource to facilitate ongoing care management.
To work in partnership with the team, patients, carers, and members of the multi-disciplinary team to offer support and advice and promote informed choice; maximizing independence and optimizing quality of life. This post holder will be required to provide clinical management, HR support, and leadership to all staff within the team.
