Northumbria Primary Care are delighted to be able to offer a Customer Care Coordinator post at Northumbria Healthcare Innovation Hub. We are looking to fill a 22.5 hour position.
The main focus of this role would be clinical coding. The successful candidate will be based at Northumbria Primary Care innovation hub at Seaton Delaval but will work as part of a team across NPC Practices providing cover during periods of leave.
Ideally, the successful candidate will have a background of working in primary care, as well as strong customer service and administrative skills.
To collect and input data from both paper and electronic correspondence to EMIS/System One, extract appropriate clinical details from medical correspondence and other relevant documents received by the practice, within set time-scale, applying practice specific/ local/national and international clinical coding conventions.
Use judgment to translate cases where the information included is not readily codable.
Northumbria Primary Care Ltd (NPC) is a pioneering partnership with local GPs and no manages several GP practices with around 50,000 registered patients in Northumberland and North Tyneside.
Whilst appointees will have a base practice within NPC, they may be asked to work at other practices across our group. We do however limit this as much as possible as we appreciate continuity is vital to effective primary care.

Be able to care navigate the patients to see the right person and the right time. The successful candidate will follow a formal training plan to ensure care navigation is of patients is completely accurately and confidently.
To provide admin support to nursing & clinical staff.
To ensure all Emergency Care documentation is coded & filed correctly, and copies are sent to relevant health care professionals.
To provide non clinical advice to patients, relatives, the public and internal & external agencies.
To receive confidential information by telephone or face to face and input accurately into both EMIS and SystemOne as appropriate. The job requires a broad range of tasks and offers general admin and secretarial support to the team.
Abstracting and analysing information from correspondence received at the practice, typically in the form of discharge summaries and correspondence from clinicians/departments out with our organization.
Reading and accurately translating clinical information into diagnostic codes and enter the patient data on to the practices clinical system (SystmOne)
To manage correspondence, secretarial duties, nursing admin support and processing patient referrals. Processing and distributing incoming (and outgoing) mail.
To provide clinical coding of all patient records ensuring all medical history is coded accurately and entered onto the clinical system some of which is sensitive.
