The Homeless Rough Sleepers Nursing Team are looking for an experienced nurse to work in Mendip to provide early intervention and prevention nursing care for people who are homeless or rough sleeping across Somerset. This includes those in hostel, supportive, or unsecure accommodation.
You will require excellent communication skills, a teamwork approach, and the ability to think outside the box to provide a high standard of nursing care, along with a full driving licence and the use of a vehicle. Main duties of the job
You will play a vital front-line role tackling health inequalities by working with one of the most vulnerable groups in the county. Working collaboratively with people and partner agencies, you will proactively engage with individuals at a range of venues in local communities, helping people to improve their lifestyle and overall health by contributing to a holistic approach that deals with the wider determinants of health. As an experienced and skilled nurse, you will provide appropriate and compassionate healthcare. You will work with the clinical lead to ensure that the team develops to meet the needs identified and undertake any relevant training required to enable them to work as autonomous competent practitioners. You will work as part of a team making every contact count, taking every opportunity to impact directly and indirectly on the health and wellbeing of people. You will be able to work under direction and as an autonomous practitioner as agreed with the service lead. You will demonstrate safe, clinical decision-making and compassionate care, including effective assessment and diagnostic skills, for people requiring care.
