Primary Key Responsibilities: The following are the core responsibilities of the Care Navigator. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels and training: Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition. Answer incoming phone calls, transferring calls or dealing with the caller's request appropriately. Maintain and Monitor all online requests/Econsults are actioned in a timely manner. Initiating contact with and responding to requests from patients, team members, and external agencies. Clinically code data on Emis Web System accurately. Patients registration, deductions, Iron Mountain tasks. Input data into patient healthcare records as necessary. Direct requests for information, i.e., SAR, insurance/solicitors letters, and DVLA forms to the administrative team. Manage all queries as necessary in an efficient manner. Maintain a clean, tidy, effective working area at all times. Monitor and maintain the reception area and notice boards. Support Care Navigators/Administrators, providing cover during staff absences. Scan patient-related documentation and attach scanned documents to patients' healthcare records and code accurately. Complete opening and closing procedures in accordance with the duty rota. Produce, maintain, and participate in the Care Navigators rota. Undertake all mandatory training and induction programmes, ensuring all mandatory training is maintained and up to date. Support the Patient Participation Group related matters. Support the health promotion lead and display promotional material on the allocated notice boards and in the waiting room. Attend a formal appraisal with their manager at least every 12 months. Contribute to public health campaigns (e.g., flu clinics) through advice or direct care. Train and support new reception staff in reception duties and tasks. Oversee reception staff annual leave ensuring that there is no clash and cut-off dates are maintained. Restock all the clinical rooms and the necessary places. Complete daily urgent tasks as set by the Clinicians. Any other duties as may be required by the Partners and the Practice manager.
