They will work alongside multi-disciplinary teams using the Prevention, Identification, Escalation and Response (PIER) model to reduce physical deterioration. They will also have responsibility for the daily management of a caseload of patients, in collaboration with the medical teams. Main duties of the job Hospital Outreach 24/7 is a multi-professional, multispecialty approach to delivering care over a 24 hour period, with the aim of improving patient safety. We are looking for a dynamic, enthusiastic practitioner who is committed to the delivery of safe patient care in the ward and other areas outside of Critical Care, to join us and be a part of our developing Multi-disciplinary team.
