Adult Access is the front door of Adult Social Care & Health which enables a clear direction for enquiries, resource, demand management, and accountability.
Adult Access has three areas of operations to enable the delivery of the wider Adult Social Care Target Operating Model. These three functions are:
* Multi-Disciplinary Team (MDT)
* Safeguarding Triage
All three functions will focus on a seamless person-centred, strength-based approach from first contact. In practice, Wellbeing Advisors, Social Workers, Social Care Assessors and Occupational Therapy will work together as a team. The aim being to prevent, reduce and delay people needing formal and longer-term care, and to provide short term reablement support, where required, to enable people’s independence.
We are recruiting 2 Senior Practitioners to the Safeguarding Team and the MDT Team. Your application will be considered for both teams. Preferences will be discussed at interview.
The Role:
* The Senior Practitioner will be providing expert oversight and management of complex safeguarding cases in the triage process.
* In the role, the Senior Practitioner will be supporting the Team Manager in handling a high volume of safeguarding concerns (approximately 250 contacts per week).
* Senior Practitioners are responsible for triaging and analysing complex cases, making informed decisions about immediate protection needs and appropriate interventions.
* Senior Practitioner will be involved in mentoring and supporting less experienced team members, enhancing overall team competence.
* You, the Senior Practitioner, will ensure adherence to best practices, legal requirements, and local policies and procedures.
* As the Senior Practitioner, you'll be liaising effectively with other agencies to ensure coordinated and comprehensive responses to safeguarding concerns.
