Children and Young People Mental Health Community Worker

To work as part of a Specialist Children and Young People's Mental Health Team to provide intensive Tier 4 support and interventions for young people and their families. The post will support Senior Mental Health Practitioners in delivering safe and effective care to young people and their families, as agreed in their care plan. The post will involve working under the supervision of a senior member of the team but will include independent work in the young person's home and other community settings. The hours of operation for the Children and Young People's Mental Health Community Intensive Support Team will be variable and will require out of hours working that will be 7 days per week.
Main duties of the job
1. To provide support for young people and their carers/families to enable them to achieve collaboratively agreed defined in their care plan.
2. To support the development of relapse prevention plans that focus on the young person and families strengths and pro-active approaches to maintaining a positive wellbeing.
3. To promote young people's social engagement, health and wellbeing and age appropriate independence.
4. To actively support young people to access appropriate local community resources.
5. To promote services users achieving their goals in terms of working towards recovery.
6. To create and maintain effective working relationships with external organisations and agencies.
7. To keep accurate and current clinical records.
8. To support the service user in care planning and promoting independence.
9. To form positive therapeutic relationships that support service users' own decision-making and build on their strengths.
10. To support service users in creating a crisis and contingency plan wherever appropriate.

Person Specification
Qualifications
* Assistant Practitioner/Nurse Associate or NVQ Level 3 qualification in a related field and equivalent training or experience in mental health or CYP emotional health and wellbeing to diploma level.
Knowledge and experience
* Knowledge of child mental health and its effects on individuals and society's response.
* Knowledge of common mental health presentations and the appropriate support and interventions available.
* Knowledge of the local area your service is based in (community groups, services available as well as local demographics).
* Knowledge of the diverse cultures within the communities across Cheshire and Wirral.
Experience
* Experience of working with young people and/or families with complex mental health and wellbeing needs.
* Experience of working effectively within a team.
* Experience of working unsupervised and using own initiative.
* Experience of clinical record keeping and using electronic systems where appropriate.
* Experience of using basic word processing and data input packages on a computer or laptop.
* Experience of prioritising workload appropriately.
* Experience of working as part of a team and independently under own initiative and seeking support as necessary.
* Experience of lone working within a community setting.
* Experience of working with CYP and Families from diverse cultures.
