We are excited to offer a permanent opportunity for an experienced and motivated Social Worker to join our Early Intervention and Prevention Team in Adult Social Care, offering 30 hours per week.

In our Adult Social Care, Early Intervention and Prevention team, you will support our most vulnerable residents in getting their lives firmly on track, using a strength-based approach and always keeping the person at the heart of everything you do.
As a Social Worker, you will have the time to build meaningful relationships with the people you work with, making a real difference in their lives.

About the team...
Our Adult Social Care service is composed of highly professional and supportive multidisciplinary teams. We have a “can do” and “no wrong door” ethos. Our teams work together as one service and go the extra mile to ensure our residents have the right support.
We work closely with health and other partners to ensure people avoid unnecessary hospital admissions. If hospital care is needed, we make sure it is only for as long as necessary and we focus on helping them return home as quickly and safely as possible.
We offer a range of preventative and rehabilitation services designed to help people remain in their own homes. Through regular multi-disciplinary forums, we ensure residents have easy access to integrated health and social care services, providing holistic support when needed.
Recognising the challenges working in Adult Social Care, we make sure our staff are well supported. You will receive regular supervision and access to training. Our focus is on supporting staff to learn, grow and develop, helping them achieve their full potential. We actively listen to social care staff about the best ways to deliver services and encourage feedback and suggestions for continuous improvement.
Key responsibilities...
* You will manage a caseload, organise and prioritise the workload effectively, with regular supervision and guidance.
* You will work as part of a multi-disciplinary team, connecting people and their families to their community and social networks enabling them to live within their own communities.
* You will take a strengths-based approach to support people to live the best life they can and reduce dependence on long term care.
* You will work within allocated resources as set out in our scheme of delegation.
* You will actively participate in your own supervision and appraisal, while also supervising and appraising allocated staff in line with the departmental supervision policy and the council's appraisal policy.
* You will take responsibility for your own learning, including mandatory and recommended training.
