We are offering an exciting opportunity for an Advanced Clinical Practitioner to join Mid-Mersey Crisis Resolution and Home Treatment Team.
This role will require expert assessment, planning, implementation and evaluation of care for people requiring urgent support with their mental health.
This post will be based at Harry Blackman House, but practitioners will cover a footprint across Knowsley, St Helens, Warrington, Halton. There may also be expectation for practitioners to support across areas of Liverpool depending on service need.
To provide strategic clinical leadership in relation to an area of specialism using the most up to date and evidence based theory
To undertake expert assessment, planning, implementation and evaluation of care for a specific group of service users
To offer expert clinical advice for service users, carers, staff and other professionals
To demonstrate advanced competence in highly complex decision making, assessment and the management of clinical needs within the specialist area of practice
To support and complement the existing medical arrangements
To work in partnership with service users, carers and other agencies to ensure effectiveness of outcomes

CLINICAL/MANAGERIAL
To work as advanced clinical lead for an area of specialist practice across a range of services within the Urgent Care.
To utilise expert mental and physical health professional skills to provide assessment, diagnosis, intervention and clinical guidance as appropriate, whilst responding to changes to the service user physical and mental and mental health
To formulate, implement and evaluate interventions with individuals and their carers where appropriate.
To make and receive referrals as an advanced autonomous practitioner
To demonstrate advanced knowledge and skills within a defined area of expertise
To critically appraise, evaluate and apply knowledge of theoretical frameworks and research evidence within a specialist area of practice
To visit service users in the most appropriate setting to meet their needs as required
Refer service users to other members of the multi-disciplinary team or specialism’s as their needs require
To use independent non-medical prescribing skills in the initiation and evaluation of treatment plans
Establish consistent and evidence based side-effect medication monitoring, acting and findings as necessary
Lead on the transition from medical led services to practitioner services within a specialist area of practice
To discharge from a specialist area of practice with an effective discharge plan of care in place
To liaise with other services as required
To undertake physical examinations of service users as required
To refer for medical intervention as the service users needs require
To make changes within the clinical practices of the specialist team based on evidence, policy and/or the identified needs of service users
To ensure that clinical and non-clinical risk is identified and dealt with appropriately. To report to the Local Division management team any risk that cannot be managed at an acceptable level without external support
To provide verbal or written reports as requested by other departments or the Local Division management team as required
To identify areas for service development in your area of responsibility
To adhere to Trust Policies reporting to the Local Division management team any failures in adherence to Trust Policy
To contribute to the development of Trust Policies and Local Division Guidelines
To maintain the high standards in clinical expertise that this role requires
To maintain, monitor and present data relating to the role
To be consistent and active in evaluating the effectiveness of the role
Participate in clinical research as necessary whilst ensuring adherence to ethical practice
To participate in service meetings
To offer and participate in regular systematic supervision
Establish priorities and organise workload to ensure effective use of time
Ensure accurate recordings of all referrals, caseload and outcome data
Ensure systematic records of treatment and progress are kept, and that written reports are provided for relevant returns
Provide statistical returns
Produce full evaluation of service and its impact on an annual basis

To attend relevant meetings, courses, seminars so as to keep up to date professionally
Analyse critically, sources of information that inform decision making in relation to non-medical prescribing
To prescribe safely, appropriately and cost effectively
To apply knowledge of drug actions in prescribing practice
To work within the legislative framework relating to non-medical prescribing
To keep up to date with developments in mental health so that their prescribing decisions remain evidence based
To be responsible for maintaining own professional qualifications and maintain a contemporary knowledge of professional practice issues
To participate as a supervisor and supervisee within the Trusts Performance Management/Supervision framework. To work towards and help others work towards goals and objectives, as agreed in PACE.
To undertake mandatory training as stipulated by the Trust
The post holder will work closely with the Consultant Psychiatrist for their specialist area of practice and other lead professionals within the Division.
