This is an exciting new opportunity within the Integrated Stroke Early Supported Discharge and Community Neuro Rehabilitation Service. We are seeking a highly motivated band 7 Nurse with a special interest in Stroke. The Integrated service provides both stroke and community neuro rehabilitation, supporting service users with a variety of complex needs.
This role requires the successful candidate to work in a clinical lead capacity in the service, developing stroke pathways but will also hold a clinical caseload undertaking specialist assessments of patients with complex and multiple pathologies using advanced, specialist clinical reasoning skills. In addition, the successful candidate will work in partnership with the existing ABI Coordinator to ensure the smooth running of the Level 2 ABI Pathway.
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Job responsibilities
* Lead a county wide Stroke Specialist Nurse pathway within the Integrated Stroke ESD and Community Neuro Rehabilitation Service to provide high-quality assessment and rehabilitation to stroke patients, promoting optimal independence and recovery.
* To support the Acquired Brain Injury (ABI) level 2 pathway in Cambridgeshire and Peterborough (C& P) co-ordinating the placement of correctly identified patients into non-NHS Facilities, overseeing regular reviews including MDT meetings and timely discharge at the end of their placement. This will include working with partners regularly including the Integrated Care Board (ICB) and providers.
* Provide clinical leadership to support the development of the Integrated Community Stroke Service (ICSS).
* Manage referrals into the service, ensuring prompt assessment and expert stroke advice.
* Hold a caseload of patients with complex stroke issues, supporting their management, recovery, and rehabilitation, while also assisting other Specialist nurses.
* Foster communication between services to ensure seamless stroke pathways between acute and community settings.
* Coordinate training and development opportunities for stroke specialist nurses and other colleagues in the MDT.

Education / Qualifications
Essential
* Degree or equivalent e.g: Diploma entry level qualification into the Nursing profession.
* Current NMC Registration.
* Professional knowledge and training to master's level or equivalent experience.
* Theoretical study and experience within a stroke/neuro setting to master's level or equivalent experience.
* Documented evidence of own continuing professional development.
* Fieldwork/Practice Educator qualification.
Desirable
* Fieldwork/practice Educator qualification.
* Member RCN.
* NMC qualification (Specialist practitioner/district nursing).
* Neurological Practice (SSNP).
* NHS Leadership qualification and experience.
