We are looking for an enthusiastic, dynamic and committed clinical pharmacist with well-developed interpersonal skills to join our innovative clinical pharmacy team at North West Anglia Foundation Trust.
Your focus will be to deliver specialised, patient centred clinical pharmacy service to our medical division, through attendance of consultant led ward rounds and facilitation of safe and rapid discharge process utilising our near patient dispensing and discharge transcribing processes. We have developed a bespoke dashboard allowing workload to be prioritised allowing pharmacists skills to be concentrated on clinical patient facing activity.

This post attracts the Trust welcome bonus of £1000 payable in one instalment on commencement. Should you leave prior to 6 months you will be required to repay the full instalment and should you leave within 12 and 18 months of employment you will be required to repay £500.
Main duties of the job
You will develop, deliver and evaluate specialist clinical pharmacy services to the Trust and provide comprehensive specialist pharmaceutical care and medicines management by the provision of safe, appropriate and effective clinical pharmacy services, ensuring compliance with current medicines legislation.
You will be supported and working with the Advanced Clinical Pharmacists and other senior members of staff to ensure the highest standards of care are continuously delivered.
You will be part of the on call/weekend pharmacists' rota and dispensary duties rota attending all the Trust hospital sites.
As part of your role, you will provide a ward based clinical pharmacy service to areas across the Trust to ensure the accuracy, legality, safety and efficacy of prescribed medication. You will ensure medicines management, reconciliation and dispensing are undertaken in line with current medicines legislation.
You will use your knowledge and expertise to educate and counsel patients and carers on the safe and proper use of medicines. You will participate in ward rounds and clinical meetings relevant to the speciality to provide pharmaceutical advice to doctors and other healthcare professionals.

* Participate in the safe dispensing and supply of medicines to in-patients and outpatients.
* Be actively involved in discharge planning and the provision of discharge medicines.
* Be jointly responsible along with other pharmacy staff for the safety and security of pharmaceuticals including controlled drugs.
* Assist Lead Clinical Pharmacists or other senior members of staff to develop Pharmacy services in line with Pharmacy and Trust strategies.
* To ensure the appropriate, safe and cost-effective use of medicines by actively undertaking individual medication review to optimise therapy.
* Participate in clinical audits within clinical areas, ensuring prescribing represents best value for money.
* Deliver and develop evidence-based guidelines and policies in liaison with Lead Clinical Pharmacists and Clinicians.
* Ensure dispensing and prescribing reflects the contents of the Trust Formulary, challenging where appropriate, and ensuring adequate documentation is provided to primary care, if agreed to proceed with supply.
* To act as an Independent prescriber when required and appropriate and if not already an NMP will be prepared to undergo the training.
* Clinical screening and authorising of homecare prescriptions, liaising with clinical teams to resolve problems and queries.
* Work in conjunction with the Pharmacy Technician Homecare Specialist to provide a comprehensive homecare service to Trust patients.
* Provide evidence-based information on medicines for Pharmacy QIPP and Drug and Therapeutics Committee.
* To register as a pre-registration tutor and participate actively in the teaching and training of pre-registration and junior rotational pharmacists, pre-registration graduates and students undertaking NVQ courses, and technical staff whilst allocated to the specialist team tutor assigned students within the department.
* Undertake mentoring responsibilities for clinical pharmacists through their rotational training programme. Provide support for the delivery of medicine related training programmes to members of the multidisciplinary clinical teams.
* Assist in the development of policies and guidelines and input into the training of staff using these documents.
* Assist in the implementation and monitoring of national guidance e.g. NICE, NPSA for the specialist area.

* Pharmacy degree from recognised university
* Registration (or eligible for registration) with General Pharmaceutical Council with registration maintained during entire period of employment
