Job summary We are looking for a Clinical Pharmacist to support a number of services being delivered across our Frailty Service Our Clinical Pharmacists work with a team of doctors, specialist nurses, pharmacy technicians and therapists providing a comprehensive geriatric assessment to patients seen inthe Jean Bishop Integrated Care Centre, care homes, our community bedded units, our hospital at home frailty virtual ward (including home visits). We currently have a vacancy for 2 days per week on a Monday and Tuesday to support our current projects above. Core Hours of Service: Monday and Tuesday 8.30am - 4.30pm Main duties of the job Experience of performing structured medication reviews is preferable but not essential. We will provide training as part of an individualised induction programme and you will be embedded within an experienced frailty team. The successful Pharmacist will have a focus on patient centered care, and an interest in frailty, they will demonstrate excellent communication and organisational skills. Applicants are welcome from all pharmacy sectors. You will receive professional support from our established Medicines Service team based in Hull. You must have an independent means of transport to travel around the geographic area our services operate across and also to work flexibly to meet the needs of the organisation.
