Job responsibilities
Applicants must have a full valid driving licence and have access to a car for work. If necessary, adjustments can be considered in line with the Equality Act 2010.
The service operates 7 days per week, 8am to 6.30pm. Full or part time (minimum 22.5 hours) roles are available. Annualised hours contracts will be considered.

Person Specification
Qualifications
* Registered General Nurse (adult).
* Educated to degree level or equivalent.
Knowledge and Experience
* Knowledge around long term conditions and their management.
* Evidence of ongoing professional development.
* Previous Community Services experience.
Technical expertise/competence
* Demonstrates an understanding of local and national issues influencing the delivery and development of health care.
* Demonstrates a knowledge and understanding of evidence-based care.
* Demonstrates an understanding of the Clinical Governance requirements.
