The Role – Resettlement & Outreach Officer
We are looking for a Resettlement & Outreach Officer to deliver high-quality, person-centred, and trauma-informed housing support services to individuals facing homelessness.
As part of our Outreach and Resettlement Service, you will be responsible for interacting with street homeless people and rough sleepers, helping them transition into permanent accommodation, improve their physical and mental health, and engage in positive economic activity. You will provide ongoing support to those resettled to help them maintain their accommodation and reduce repeat homelessness. This will involve:
• Providing housing advice and support to homeless individuals and signposting
them to relevant services and agencies.
• Recording client journeys, maintain accurate records, and develop support
plans that assist clients in maintaining independent living.
• Engaging with statutory and voluntary agencies, attending meetings and
coordinating support to ensure effective service delivery.
• Advocating on behalf of clients, liaise with agencies, and provide intensive
support to ensure successful resettlement.
• Supporting service users with property induction, risk assessments, and
ensuring long-term occupancy sustainability.
• Assisting in providing accurate statistical information and reports for funders
and stakeholders.
• Understanding and adhere to safeguarding procedures, escalating concerns
appropriately.

What We’re Looking For
• A compassionate and proactive individual with experience working with
vulnerable people, including rough sleepers and those suffering from mental
health issues or substance misuse
• Strong communication skills with the ability to work in a collaborative, multi-
agency environment.
• Experience in case management, risk assessments, and understanding of
housing legislation.
• An understanding of safeguarding principles, benefits systems, and the support
needs of vulnerable individuals.
• Ability to work independently, manage a caseload effectively, and respond to
high-risk situations confidently.
Essential Skills & Experience:
• Either have the ability to hold at least basic conversations in Welsh (although
you may struggle to keep up fully) or be currently learning / willing to learn
Welsh
• Experience of working in a client-centred environment, supporting individuals
with their personal and housing needs.
• Demonstrable experience in housing management or support services,
particularly for vulnerable individuals.
• Strong problem-solving abilities, attention to detail, and the ability to work
efficiently to deadlines.
• A positive, can-do attitude with a passion for helping people.
• Full UK driving licence with access to own transport (with business insurance).
