Job Summary
CDDFT Community Respiratory Nursing Service is pleased to advertise a rare, exciting opportunity for 2 permanent Community Respiratory Care and Support Workers predominantly working within the Durham Dales, Sedgefield and Darlington localities, based at Innovation House in Bishop Auckland.
The successful postholder will deliver and support respiratory care and education to respiratory patients including diagnostic spirometry performance, in out-patient clinics and their own homes, supporting the specialist respiratory nursing service.
The successful candidate will utilise their organisation and communication skills to achieve best patient outcomes whilst being well supported by expert respiratory specialists to develop a range of competencies in delivering excellent respiratory care and support for this group of complex patients.
The hours for this post are Monday-Friday between 8:30 and 6pm; flexibility can be negotiated in regards to days and hours worked.

Main Duties of the Job
DIMENSIONS OF THE JOB
1. To work under the direction of the registered community respiratory specialist nursing team.
2. To assist in the delivery of respiratory care.
3. Provide specialist education to patients regarding healthy lifestyle, inhaler technique and self-management plans.
4. Care delivery in a community clinic or patients' home environment.
5. Maintain accurate records.
6. Take responsibility for medical devices and service equipment registers.
7. Participate in service improvement and developmental initiatives.
8. To work within the multi-disciplinary team including therapy, social, housing, well-being services, administrative and support staff.
9. No requirement to manage/monitor budget.
About Us
We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community-based settings and providing care in patients' homes.

Job Description
Main Duties And Responsibilities
Management and Leadership
1. Contribute to creating improvement, transformation and innovation within the service in order to promote best practice.

Clinical Responsibilities
1. Co-support a caseload of respiratory patients, under direction of the community respiratory specialist nursing team, following assessment, across the community respiratory specialist service, within clinic and home environments.
2. Perform diagnostic spirometry.
3. Support respiratory patients and family/carer involvement and participation.
4. Accurately record clinical data and understand its significance including heart rate and rhythm, respiratory rate, pulse oximetry, peak flow rate, and blood pressure.
5. Complete effective record keeping which demonstrates the care given, as well as patient outcomes in accordance with Trust policy and legal requirements.
6. Understand the benefits of non-pharmacological interventions for respiratory illness/conditions including smoking cessation, weight management and nutrition, and physical activity.
7. Understand motivational skills and empathetic communication.
8. Promote health and healthy informed choices.
9. Review smoking status, provide very brief advice and refer to appropriate smoking service.
10. Review and train inhaler technique to patients, carers and other health care professionals.
11. Have awareness, knowledge and implementation of safeguarding processes in relation to children, vulnerable patients and carers.
12. Support education of patients and carers and students, and other staff as appropriate.
13. Have awareness of the importance of research, audit and quality improvement.
14. Participate in and contribute to all activities associated with the team's response to current trends and changes within the NHS.
15. Promote and support effective implementation of all Organisational Policies and Procedures, and practice in accordance with.
16. Liase with multi-disciplinary teams, referring as appropriate.
17. Support and promote a team culture which actively encourages empowerment, thereby supporting and facilitating change.
18. Support the provision of a high quality, value for money and effective service within available resources.
19. Possess good IT skills and use of systems.
Educational Responsibilities
1. Support training for the multidisciplinary team in the management of respiratory disease.
2. Regularly attend and participate in training, peer support and clinical supervision.

* Ability to observe and record physical and social changes of a patient and report immediately.
* Phlebotomy or willingness to work towards.
* Ability to work independently and act accordingly in an emergency situation.
* Able to prioritise work on a daily basis to meet service demands.
* Confident in the use of email and basic IT skills.

* Basic understanding of respiratory anatomy, function and conditions.

Qualifications Essential
* Health care certificate or willingness to complete in the first six months of employment.
* NVQ3 or relevant equivalent experience.
