An exciting opportunity has arisen for a Care Home Nurse at Nunwell Surgery in Bromyard.
We are seeking applicants who are clinically experienced, motivated, enthusiastic, and able to work autonomously and safely within the limits of their own competence and enjoy working collaboratively within a team.
The role's main focus will be to provide high quality, planned and proactive care for care home patients and to avoid unnecessary hospital admissions.
Main duties of the job
The Care Home Nurse will maintain a high profile professional leadership role across the Practice inclusive of clinical governance and a commitment to high-quality, evidenced based practice. They will ensure that the Enhanced Care in Care Homes Framework and Primary Care Network (PCN) Enhanced Health in Care Homes Directed Enhanced Service (DES) is translated into the provision of evidenced based practice within Care Homes and scope of own practice. Care Homes within the Practice catchment area include Nursing and Residential.
The Care Home Nurse will build on and strengthen the link between all health and care professionals delivering care and support to care home residents inclusive of colleagues within General Practice and Community Services, Social Care, Care Home Teams, and the Voluntary Care Sector.
The Nurse will work collaboratively and proactively within a multi-disciplinary team (MDT) to ensure care and support for care home residents is seamless and timely. The role is not expected to receive referrals for same day urgent care as it is proactive rather than reactive in nature; however, there will be a need to respond within an MDT approach to coordinate ongoing care resulting from a change in patient condition.

We are a GMS practice with 9500 patients. The team comprises 6 partners and 3 salaried GPs, an excellent nursing team including two nurse practitioners, two practice nurses, and 2 skilled health care assistants, trusted and hardworking reception and admin teams working alongside our busy dispensing staff and practice pharmacists.
Job responsibilities
* Working within the Care Home Team to provide patient-focused care.
* To support the practice with ongoing management of care home patients by reviewing patients and amending Personalised Care & Support Plans and RESPECT forms.
* To support care home patients with anticipatory care discussions and planning.
* To provide acute medical treatment for care home patients with undifferentiated and undiagnosed problems seeking a primary health care service including telephone triage, face-to-face consultations, and home visits.
* To assess and manage patients with undifferentiated and undiagnosed health care needs including investigating, treating, and referring patients as appropriate.
* Independently prescribe (where applicable to the role) and conduct medication reviews, review appropriate medications in accordance with local policies and evidence-based medical practice, and work collaboratively with the wider team to support patients and to promote medication adherence.
* To order investigations, follow-up results, and arrange management including seeking advice where appropriate.
* To work with a multidisciplinary team including community and voluntary sector to improve care home patient support.
* Actively take a personalised care approach and population-centred care approach to enable shared decision making with the presenting person.
* Attend ward rounds and MDTs.
* A prescribing qualification for this post is essential and an interest in frailty would be beneficial.
Person Specification
Qualifications
* RGN current NMC registration with a minimum of 4 years experience in primary care or community setting.
* Safeguarding adult and children Level 3 (or willing to complete).
* Knowledge of Mental Capacity Act.
Experience
* Experience of working in Primary Care in an autonomous role.
* Experience of working in a multi-professional environment.
* Experience of Advanced Care Planning and Palliative Care.
* Experience of working under pressure.
* Experience of EMIS WEB clinical system and using templates.
* Knowledge on Quality Outcomes Framework.
