The Role: You as the Social care Assessor will be to assess and where necessary, provide reablement or short-term support to individuals to promote their independence. Where long term intervention is required, a robust assessment will be completed to enable a smooth transition to the Locality Teams for on-going intervention. Your role as Social Care Assessor will involve working with a wide range of cases for all adults from the age of 18. As the Social Care Assessor you will effectively manage a varied caseload with appropriate supervision, guidance, and support, reflecting the function of the team and Adult Social Care in accordance with policy, procedures, guidance and legislation. As part of your role as a Social Care Assessor you will complete Reablement Assessment, Your Needs Assessment, Risk Assessment, Mental Capacity and Best Interest assessment, completing Reviews, as well as any other relevant assessment in relation to a person’s needs, as well as be able to commission services such as Domiciliary Care packages, Day Centre placement. Carers support including carers assessments. Assistive technology, minor adaptations etc Social Care Assessors within the Multi-Disciplinary Team will also include attendance of the daily huddle, participate in fortnightly Duty responsibilities and work together with social workers, Senior practitioners and Team Manager. Representing the Adult Access Team when required at any external event Through every interaction, you the Social Care Assessor will apply the Prevent, Reduce, Delay principles of the care act to promote independence and wellbeing.
