YICT supports patients to remain independent in their own homes, while continuing with healthcare assessments, rehabilitation, and recovery. We also provide an urgent crisis response supporting patients and preventing unnecessary hospital admissions.
You will work closely with a team of healthcare professionals to support patients' individual needs and promote patient health and wellbeing.
Main Duties of the Job
1. To provide first visit assessments of patients in their own home, and ongoing care and support to the individual.
2. Assist patients to meet their basic needs, including prompting and administering medication as required, supporting with dietary needs, maintaining personal hygiene, and using toilet facilities as appropriate.
3. Perform physical observations on patients using the necessary equipment (blood pressure monitors, blood sugar, and pulse oximeters) when appropriate or when advised to by a manager or member of the nursing team.
4. Ensure patients' comfort, safety, and dignity at all times, respecting privacy and ensuring confidentiality.
5. Assist and support patients and carers as appropriate.
6. Ensure referrals to other agencies are made and effectively and efficiently signpost patients to relevant agencies when appropriate.
7. Ensure relevant information, including all changes in a patient's condition, is passed to the registered nurse on duty. Ensure efficient handover of visit allocations with Healthcare Support colleagues.
8. Ensure accurate records are maintained in a timely manner including discharging and archiving patient paperwork, completing Healthcare Support visit data and CQC documentation.

We deliver Urgent Care services in collaboration with the hospital and GP practices in York, Scarborough, Whitby, Malton, and Selby. We also provide community-based frailty care, which focuses on treating people in the community where we can. We aim to shift services into the community where possible, for example, our community diagnostic centres run in partnership with the Acute Trust.

About You
1. With excellent communication skills, you will have the ability to build a professional rapport with patients, being able to calmly and professionally identify and deliver patient needs.
2. A team player, you will work effectively and professionally within the wider YICT team and also have the ability to work independently.
3. You will have a can-do attitude and be able to react to challenges or setbacks in a positive manner.
4. You will hold a full UK driving licence and have access to a vehicle.
5. Full training will be given; we invite applicants with no experience in healthcare.

£12 an hour, £13.50 for weekends and bank shifts.
