Are you passionate about making a meaningful difference in patients’ lives? As a Nurse Assessor/Trainer, you’ll play a key role in supporting individuals with complex health needs to achieve the care and support they deserve.
Partner2Care is a clinically led assessment and brokerage and support service supporting individuals who have chosen to have a Personal Health Budget (PHB).
You will assess and develop clinical Personal Health Budget (PHB) /Personal Budget (PB) care plans for patients who often have complex health needs and whose care has been agreed within their care plan with Integrated Care Board.
As a Nurse Assessor/Trainer, you will work with a Brokerage and Support Officer and Trainer to ensure that the care and support will meet their health and well-being needs safely and legally and incorporate care support from both formal and informal care providers.
You will work across multi-disciplinary and multi-agency parties to ensure that safe and effective care will meet needs and outcomes and are within the allocated budget.
Main duties of the job
In this role, you will:
* Assess and Develop Tailored Care Plans: Work closely with patients, families, and the Integrated Care Board to create Personal Health Budget (PHB) and Personal Budget (PB) care plans that are personalised, effective, and aligned with their health goals.
* Collaborate Across Teams: Partner with a Brokerage and Support Officer and Trainer to ensure care is delivered safely, legally, and holistically, incorporating support from both formal services and informal care networks.
* Drive Positive Outcomes: Engage with multi-disciplinary and multi-agency teams to deliver care that is safe, effective, and within budget while meeting each individual’s unique needs and desired outcomes.

Qualifications
* NMC Registration
* Experience post qualification, evidence of ongoing studies/research/learning
Knowledge
* Evidence of assessment, patient-centered care planning and evaluation skills
* Experience of working with adults with complex health needs and their carers.
* Experience of managing a defined caseload.
* Experience of commissioning and budget management/ working with schedules and contracts or willingness to learn
* Knowledge of Social and Health care sectors
* Understanding of the delivery of care home packages for those with complex needs.
* Ability to use appropriate electronic systems and software packages e.g., Microsoft packages, EMIS etc
* Understanding of Department of Health’s of National Framework for NHS Continuing Healthcare and NHS –funded Nursing Care personal health budgets Personal Health Budget and Personalization
* Understanding of the role of Commissioning and changes in the NHS.
You must have appropriate UK professional registration.
