This is an exciting opportunity to work in the Coventry Community Mental Health Team. You will be working alongside Coventry and Warwickshire Partnership Trust colleagues to provide a seamless service to citizens with severe and enduring mental health challenges. The team is a multidisciplinary service, and we work closely with nurses, occupational therapists, psychologists, and psychiatrists to deliver person-centred care to our citizens. Our vision is 'Providing support, in the least intrusive manner possible, based on the assets, resources, and abilities that are available to people.'
Included but not limited to:
* Undertake assessments under the Care Act 2014 to establish and identify unmet social care needs with a focus on strengths and recovery.
* Implement care and support plans for citizens and carers which are proportionate and person-centred. Be involved in reviewing and evaluating the effectiveness of care plans and services.
* Be a proactive member of the team, offering support to colleagues through attending daily handovers and offering ad hoc support at times of crisis for citizens.
* Attend multi-disciplinary meetings whether within the team or external teams i.e. inpatients.
* Maintain effective liaison with statutory, voluntary, and independent sector agencies on behalf of service users to assess and co-ordinate a range of specific services in respect of care management to meet their needs.
* Attend and participate in regular Team Meetings.
* Foster professional development by participating in training programmes, courses, and seminars in consultation with and the approval of the Team Manager.
* Participate in regular supervision.
* Maintain prompt accurate records and other documentation relating to work with service users in accordance with approved policy and procedures.
This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).
