The shift pattern for this position is Mon-Fri 9-5.
The Care Home Collaborative (CHC) offers an opportunity to work in partnership with Care Homes and the Health and Social Care Partnership Teams (HSCPs) across NHS Greater Glasgow and Clyde to support improvements in palliative and end of life care for residents and their families. The CHC team are looking to recruit a motivated and passionate palliative care nurse specialist to join the team.
The post holder, as an expert in all aspects of Palliative Care, will work in partnership with care home teams to identify training needs and provide access to education and training to support the application of evidence-based best practice as it relates to Palliative Care. The post holder will work collaboratively with multiple key stakeholders to identify, initiate, and implement quality improvement methods to support improvements in palliative and end of life care.
The post holder must be a first level registered nurse, with at least 2 years’ experience in a palliative care nursing role and educated or working towards relevant Master’s level education. This post is currently based at Clutha House; however, there is a planned relocation of the team in December 24. A driving licence and access to a car is essential to fulfil this Boardwide role.
